About the Forms
The forms should automatically open in the appropriate software program when you click on the links: either Adobe Acrobat Reader or Reader/Writer (at least one of which should be loaded on most, if not all, CON computers) or Microsoft Word, depending on the form. For the PDF coversheets, you can either save the uncompleted versions to fill out with Adobe Acrobat later (using File -> Save As) or fill them out immediately onscreen. However, you will not be able to save the completed form unless you have the full version (reader and writer) of Adobe Acrobat. If you do not have the full version, once you have filled out the form onscreen, print a hard copy immediately (without trying to save first) for mailing and for your records.
You can use your tab key or your mouse to move from one form field to the next in the PDF files. Each of the name fields (first, middle, last, and degree) is a separate entity (i.e., you can tab between them). Note: If your title is too long for title field provided, please contact the ORS grants specialist in the ORS who will revise the form for you.
ORS Support Request Form
Use the support request form to request ORS assistance with research-related work from the development of a proposal through the dissemination of the results. Download the form below or obtain a hard copy from the ORS office. You may submit your request via e-mail to the ORS Office Manager (Laurie Rinfret) or deliver it to HPNP 2201. Please be certain to provide a thorough description of the work being requested and any relevant deadlines and attach/include all necessary documents at the time of submission.
