Please insert the exact name of your scholarship, located on your official notification of scholarship award. If awarded more than one scholarship, please submit separate online thank you messages for each.
Say Cheese(Required)
A high res photo is needed for your postcard. To be sure it is high res, make sure you can enlarge/zoom without pixelation. Tips: Ask someone to snap a pic of you sporting your new white coat, the pic should feature you, don’t try to fit a large background in your shot.
Max. file size: 125 MB.
Enter three paragraphs expressing your message of appreciation to the donor of your scholarship. Think as if you have a personalized piece of stationary in front of you. Ideas of what to include: acknowledge the support from their fund by name; how this support impacts you; why you chose nursing; your nursing aspirations; feelings of appreciation for their kindness/belief in you/generosity; say “thank you.” Around 225 words are suggested to fit properly on the postcard. Do not include a salutation or signature block in either your word count or this box, as we will already have these formatted separately. IF YOU HAVE BEEN ASKED TO WRITE A LETTER – please enter “see letter” in this box, and upload your letter in the next section of this form.
Max. file size: 125 MB.
For select funds, we require a full letter, in lieu of the shorter postcard message. If your award notification email/letter instructed you to submit a full letter, please upload it here. MS Word is the preferred format.
